In recent times, there have been concerns regarding patient safety while in the care environment since patients are exposed to various hazards and events that can endanger their lives and also reduce patient satisfaction. Bodies like the World Health Organization have made several efforts to improve patient safety, and therefore, patient safety is being used as the fundamental care quality dimension (Mieiro et al., 2019). One of such concerns is medication errors which have largely been referred to as any preventable event that can lead to or cause an inappropriate use of medication or patient harm while the medication is the control of the consumer, patient, or healthcare professional. Therefore, the purpose of this paper is to formulate an annotated bibliography on medication errors and the current efforts to address the situation.

Interest in the Topic and Professional Experience

As a professional, I believe in the safety of patients and that patients need to be satisfied with the nature or kind of patient care offered in the care setting. The implication is that the nurses and other healthcare professionals need to do their absolute best in ensuring that the patient gets the best. This topic is of interest since medication errors are largely preventable. Recent findings indicate that medication error is among the primary causes of patient mortality and morbidity and that in the USA alone, medication errors cause up to 7,000 deaths every single year while also leading to substantial intangible and tangible costs (Mieiro et al., 2019). Therefore, efforts are needed to explore more robust strategies to control and prevent medication errors. I have had a professional experience with medication errors, where a former staff member was involved in a medication error involving preparation and dispensing leading to medication overdose. The error negatively impacted the Patient’s life and had to be admitted to the emergency department to help negate the impacts of the overdose. The facility was later served with litigation papers followed by a costly court case.

Medication Errors

The promotion of patient safety is an area of considerable focus in nursing practice. Patient safety entails the provision of high quality care to the patient with the focus of eliminating any possible harm. Despite this focus, the issue of medication errors continues to affect the quality and safety of care that patients receive in health organizations. Medication errors cause significant harm to patients, distress to healthcare providers, and high cost to healthcare organizations. Medication errors occur at different points of care that include dispensing, ordering, documenting, transcribing, administering, and monitoring. A number of factors contribute to medication errors. The factors include distractions, distortions, illegible writing, burnout, and high workload.  Nurses and other healthcare providers have a critical role in preventing and reducing the risks of medication errors. Nurses adopt interventions that increase their situational awareness to the risk factors for medication errors and advocate for safe practices in nursing. Therefore, organizational structures should systems should support patient safety by minimizing the risk of medication errors and other related safety concerns.

As a future nurse professional, I am responsible for the ensuring that safe and quality care is provided to patients that I serve. I have the responsibility of minimizing the risks of medication errors in caring the populations that I will serve as a nurse. I also have the responsibility to ensure the creation of workplace culture that promotes and protects patient safety. Patient safety culture is achievable through training healthcare providers and developing guidelines that support safety in care. Therefore, I believe that I have the critical responsibility of addressing the factors that contribute to medication errors as a way of promoting safety and quality in healthcare.

Identifying Academic Peer-Reviewed Journal Articles

The peer-reviewed journals for this research were obtained from a number of databases. The databases included Google Scholar, Pubmed, Embase, and ProQuest. The search of the needed sources of data used specific keywords that included medication errors, medication administration, medication safety, medication errors risk, and preventing medication errors. The search was limited to only scholarly and peer-reviewed journals. The selection of articles was also done by considering their relevance to the focus of the research. The selected articles must have been published within the last five years to increase the relevance and accuracy of the obtained data.

Short Description:

A medication error is a preventable adverse effect of a patient taking the wrong medication or dosage, whether or not it is evident or harmful to the patient. Medication errors can be a source of serious patient harm, including death.
