To provide the highest quality of care, the World Health Organization defines interprofessional collaboration as “multiple health workers from different professional backgrounds working together with patients, families, caregivers, and communities.” More than just information sharing and effective communication between nurses and doctors are important aspects of interprofessional collaboration. All care team members must interact with the patient and with one another, and leaders must put their egos aside for the patient’s benefit. Through the use of audio and video communication to monitor the patient, the telehealth service promotes remote collaboration and communication amongst all parties involved. The video addresses issues of remote Collaboration in the Evidence-Care scenario based on the Villa Health Remote scenario.
Evidenced – Based Care Plan
The first nursing diagnosis is imbalanced nutrition, related to the patient’s chronic malabsorption and evidenced by a decrease in the subcutaneous tissue and low weight for the age of 20.7 pounds. The evidence-based practice recommends oral supplementation of pancreatic enzymes, administration of all fat-soluble vitamins and meals with enzymes, and the use of a high-calorie diet. (Kapnadak et al., 2020). The pancreatic enzymes should be administered before all meals. Remote collaboration with the family includes the development of a diet plan with the dietician, frequent weight monitoring, and regular glucose measurements.
The second nursing diagnosis is the caregiver’s lack of knowledge related to the first exacerbation, as evidenced by Caitlyn’s mother’s uncertainty over care procedures. The evidence-based intervention will involve the caregiver in hands-on training. Training parents of children with complex diseases can improve their knowledge of how to give their children the care they need (Kapnadak et al., 2020). Therefore, in light of this peer-reviewed study article, Caitlyn’s medical team should give the primary caregiver comprehensive cystic fibrosis care training. The caregiver is informed that there is no cure for Cystic fibrosis currently. Intestinal blockage prevention, lung infection prevention, lung mucus removal, nutrition improvement, and dehydration risk reduction are the main targets of treatment. Clients are living longer, with many surviving into their 40s and 50s, thanks to careful self-management and medical attention.
The third nursing diagnosis is impaired gas exchange related to airway obstruction by secretion evidenced by dyspnea. Collaboration with respiratory care personnel is needed in the institution of Bilevel positive airway pressure noninvasive ventilation recommended for patients with severe lung disease and a superimposed acute illness or assistance with nighttime ventilatory needs. Remotely, the parents should allow for sufficient relaxation in between daytime activities, with little disruption to sleep at night.
Application of an EBP Model to Develop the Plan
Caitlyn’s recovery was aided by the John Hopkins EBP model after a thorough evaluation of her health status. The John Hopkins Model is more universal since it includes procedural steps including issue identification, best evidence appraisal, and best evidence practice. Barriers to Caitlyn’s remote care collaboration include living in a place with little access to healthcare, Caitlyn’s parents’ separation, potential financial issues, potential comorbidities like DIOS, and the requirement for caregiver help. In the cycle of collaboration, effective leadership polishes a remote team’s success. To tackle interdisciplinary concerns that can impede development, the care cooperation plan must include a transformational leadership strategy. The case study’s care cycle served as an example of how the transformational leadership approach was used (Valizadeh et al., 2020). In a care collaboration plan, efficient meetings and communication can also help you deal with transdisciplinary difficulties. Effective communication makes team members more cooperative, facilitating the accomplishment of important goals.
Conclusion
It is still a problem in healthcare to give the right care to the patient population, especially to people in rural areas who have chronic conditions like cystic fibrosis. Despite medical technology developments, patient factors largely affect the implementation of care collaboration. To improve treatment for patients receiving remote care, healthcare practitioners are collaborating with health informatics to incorporate evidence-based care into emerging technology.
