Discussion 1: Quality and Safety In Healthcare and Nursing Practice Paper

Quality and Safety are fundamental concepts in healthcare and nursing practice since they consolidate diverse facets of care delivery. According to Nash et al. (2019), the primary tenets of care quality and safety include timeliness, effectiveness, efficiency, equity, and patient-centered. The interplay between these aspects of care quality and safety forms the basis of all concerted efforts of providing meaningful and reliable nursing practices amidst complexities in healthcare systems. For example, various demands confront healthcare professionals, including the calls of leveraging nursing informatics and data to improve care, the need to accommodate the widespread use of technology, the overarching objective of averting harm and health threats such as diseases, and the obligation to provide dignified, culturally responsive care to a highly diverse patient population.

As a DNP-prepared nurse, I have first-hand experience with different aspects of healthcare quality and safety. An example of my previous experience with quality and safety entails a scenario where I collaborated with other healthcare professionals such as nurse practitioners and medical assistants to deliver care to an older adult grappling with heart disease. The patient needed emergency care but had a do not resuscitate order that barred us from conducting cardiopulmonary resuscitation. As a DNP-prepared nurse, I had to establish the balance between the four bioethical principles of beneficence, non-maleficence, autonomy, and justice. In essence, I perceived the need to convince his family members about the importance of CPR in incidences of stroke or heart failure. Fortunately, we established a consensus and conducted CPR that saved his life.

From this experience, I realized that DNP-prepared nurses should demonstrate leadership skills, including showing the direction of nursing practice, making informed decisions, and encouraging teamwork. Notably, my experience resonates well with the intention of the American Association Colleges of Nursing (AACN) to establish a doctor of nursing practice (DNP) degree in 2004. According to McCauley et al. (2020), the AACN introduced the DNP degree for advanced practice registered nurses (APRNs) to enable them to bridge the chasms in nursing practice and leadership. In this sense, DNP-educated advanced practice registered nurses (APRNs) have a unique combination of clinical expertise and change management skills that position them at the pole position in advancing the tenets of care quality and safety.

As Rivaz et al. (2021) contend, a nursing doctoral degree is essential in promoting autonomy, professional development, and professional positions in the leadership, research, and education levels. In essence, DNP-prepared nurses are responsible for providing leadership for evidence-based practice, influencing health policies, promoting interdisciplinary collaboration, incorporating creative clinical reasoning skills, and anchoring the tenets of translating scientific and theoretical knowledge into practice for solving problems and improving health outcomes. As healthcare systems continue to evolve, it is now imperative that DNP-prepared nurses demonstrate awareness of dynamics in healthcare by assisting organizations to implement change initiatives by evaluating the cost-effectiveness of patient care strategies and coordinating care with quality improvement teams (Rivaz et al., 2021). Finally, DNP nurses are responsible for actualizing the six concepts of healthcare quality and safety proposed by the Institute of Medicine (IOM) in 2001. These components include process safety, care timeliness, efficiency, effectiveness, equity, and patient-centeredness. Undoubtedly, these aspects establish the need to shift from conventional professional-led care to a more holistic patient-oriented care.

Quality and safety measures are integral components in healthcare. According to Nash et al. (2019), “Around the end of the twentieth century and the start of the twenty-first, a number of reports presented strong evidence of widespread quality deficiencies and highlighted a need for substantial change to ensure high-quality care for all patients” (p. 5). Understanding the prominence of error, it is important to consider your role as a DNP-prepared nurse.
For this Discussion, take a moment to consider your experience with quality and safety in your nursing practice. Reflect on your experience and consider how your role may support quality and safety measures.
