Week 1: Developing a Focus in Your Specialty Area
As a nurse and a doctoral student, you have already developed a set of skills and the expertise that have enabled you to contribute to the nursing profession and to those you serve. Yet, your call to advanced practice warrants a deeper understanding of issues and approaches, particularly in your area of specialty practice. To practice at the highest level you must be able to analyze a health care problem using reasoning, theory, research, and experience.
As you move forward in this course, you will create an Evidence-Based Practice (EBP) Project, in which you will evaluate the scientific foundations of a health care issue in your specialty area, apply effective practices, identify and evaluate outcomes, and develop new practice guidelines. This week, you will become familiar with the requirements of the EBP Project, which will be completed as part of your practicum experience. To help you prepare for your practicum, this week you reflect on your own potential for professional growth and create learning objectives to guide your experience. The EBP Project must be completed during this practicum experience and is not the same as your DNP project that you are developing with the guidance of your DNP faculty chair and committee in the mentoring courses. Discussion: Developing an Evidence-Based Practice (EBP) Project Question
Consider issues related to your nursing practice. What stands out to you about your specialty area? What questions or concerns do you think are especially in need of attention? Why? How would you most like to contribute to the knowledge base of your profession
The EBP Project provides a significant opportunity for furthering the nursing profession. Formulating a specific, applicable question is a necessary first step for developing your project. The question defines the focus of the EBP Project and guides your selection of research methods and tools. Think about aspects of your specialty area, and begin to formulate a question that relates to your nursing practice.
The primary purpose of this Discussion is to provide a forum in which you and your colleagues can support each other in cultivating a practice-related question. NURS 8410 – Best Practices In Nursing Speciality – Weekly Discussion Essays.
To prepare:
- Develop your EBP Project question using the PICO (patient group, issue or intervention, comparison, outcome) method as described in Chapter 2 of Clinical Research for the Doctor of Nursing Practice(p. 29) and other resources. Depending on your specialty area, you may modify the PICO method as necessary to be appropriate for your target population.
- Begin to complete the “Literature Review Matrix,” reviewing at least four to six research articles relevant to your specialty area and your EBP Project question. Note any areas of difficulty that you encounter, as well as any strategies or insights that might be beneficial to others.
- Reflect on the information presented in the McCurry, Revell, and Roy article listed in the Learning Resources.
- What questions or concerns, if any, do you have about how to develop your project question and/or expand your expertise in your specialty area?
By Day 3
Post a cohesive scholarly response that addresses the following:
- Share your project question, as you have developed it thus far.
- Discuss salient elements that inform your project question: theory, current research, nursing practice, and specialty knowledge.
- Pose any questions or concerns you may have related to developing your project question or literature review. You may also include any other issues related to expanding your expertise within your specialty area.

As a part of your practicum experience in this course, you will engage in a small-scale Evidence-Based Practice (EBP) Project. This project will enable you to apply many of the concepts presented in the Learning Resources; it provides an authentic learning experience to enhance your specialty area knowledge and support your growth as a scholar-practitioner

The document includes due dates. You do not need to submit anything this week, but begin thinking about and planning for this project as soon as possible. You may wish to discuss it with your Practicum Mentor as well.
Practicum: Planning Learning Objectives
The practicum experience allows you to develop and expand your advanced nursing knowledge and skills at the aggregate, organizational, or policy level. During week 1 you will submit two to three individual learning objectives that you wish to achieve during this practicum experience. You should prepare a set of measurable learning objectives, using Bloom’s Taxonomy. Your practicum objectives should reflect your interest in expanding knowledge. They are not to include activities related to your DNP project. These objectives will be identified during your mentoring course (NURS 8700P, 8700, 8701). Practice experiences should be designed achieve specific learning objectives related to the DNP Essentials and specialty competencies (AACN, 2006). You can have more than one objective for each time interval.

Week 2: Applying Complex Reasoning Strategies
You likely make countless decisions in your day-to-day practice—many with significant consequences. How do you gather and assess information? Evaluate its significance? Weigh possible actions? As a DNP-prepared nurse it is essential to engage in self-reflection and further your awareness of how your decision-making processes and use of professional judgment can promote the application of effective practices in your specialty area.
Reasoning strategies guide current practice as well as the development and adoption of new, innovative approaches to improve practice in the specialty areas—a topic that will be explored in depth during upcoming weeks of this course.
This week you begin to develop your small-scale Evidence-Based Practice (EBP) Project in your practicum setting. In Application 1—the first in a series of assignments in which you will convey your insights and analysis of the EBP Project—you are asked to identify an issue related to practice in which an outcome is different from what would be expected according to the research literature. This issue provides the foundation for future development of the project.

In many of the courses in the DNP program, you have been asked to analyze research literature when exploring issues in health care. Reflect now on an issue in your practice area that has different outcomes from what is supported by the literature. For example, the literature evidence notes that nosocomial infections are reduced when procedures such as hand washing, glove use, and isolation strategies are implemented. In your clinical area, you have implemented these strategies and you are not seeing a reduction in nosocomial infections.
For Application 1, you select an issue that will serve as the launching point for your Evidence-Based Practice (EBP) Project.

Discussion: Quality Improvement and Patient Safety Initiatives
The vice president of nursing, Dr. Sasha Mendez, has had a lot of complaints from patients and their families about the difficulties they experience navigating the system at her organization; e.g., going from the E.R. to admissions. Dr. Mendez’s nursing specialty area is psych/mental health. As such, she is very concerned about the stress this is causing the patients.
What are the macro systems that are affecting patient satisfaction at Dr. Mendez’s facility? What micro systems should she also analyze to look at how the patient moves through—from entry, to possible admission, to possible discharge? How do these systems relate? What impacts each system?

It is important to disseminate and share the information gained from a translation-of-evidence project to enhance the body of knowledge within the profession, thus furthering the science of nursing. Yet how does one prepare to disseminate research findings? What is required to engage in this form of scholarship?
This week, you assess the role of dissemination in scholarship. You also complete your practicum experience and submit your time log and final journal entry.

Discussion: Defining Scholarship
A hallmark of your education at Walden University is your development as a scholar-practitioner. As a doctoral student, you have been actively engaged in scholarly activities that support your growth as a scholar-practitioner. As you continue to develop, implement, and evaluate your EBP project, you will be applying scholarship as you translate research into practice. A key element of scholarship is dissemination.
As you prepare for this Discussion, consider the role of evaluation and dissemination in scholarship. If research is not disseminated, can it be considered scholarship?
To prepare:
- Reflect on the definitions and criteria for scholarship presented in the Learning Resources.
- Consider AACN’s definition of scholarship and their vision of the role of the DNP-prepared nurse in furthering nursing scholarship.

Post a cohesive scholarly response that addresses the following:
- What is the role of the DNP-prepared nurse in scholarship? Provide at least two specific examples.
Read a selection of your colleagues’ responses.
By Day 6
Respond to two of your colleagues in your small group in one or more of the following ways:
- Ask a probing question, substantiated with additional background information, evidence, or research.
- Share an insight from having read your colleagues’ postings, synthesizing the information to provide new perspectives.
- Offer and support an alternative perspective using readings from the classroom and/or from your own research in the Walden Library
- Validate an idea with your own experience and additional research.
- Make a suggestion based on additional evidence drawn from readings or after synthesizing multiple postings.

- Background, Purpose, and Nature of the Project (2 points)
Describe the larger problem or issue you explored in your DNP Project. Identify your practice-focused question. Explain how this problem or issue affects nursing practice and the overall health care system, and describe how your DNP Project explored possible strategies for addressing the problem or issue. - Evidence (2 points)
Identify the practice setting and context in which you conducted your project, a summary of the evidence supporting the project, and the process for implementation and evaluation.
- Presentation of Results (2 points)
Explain the results of your DNP Project and recommendations for change that you made to the organization.
